Note that some forms provide "digital signature" fields. You will not be able to type in these fields. If you are familiar with digital signature technology and are comfortable collecting the required signatures this way, feel free to do so. The form may then be submitted electronically. Otherwise, the form must be printed to allow for a physical signature, and then mailed to the OPC office (or scanned and e-mailed).
